Design Request: Information Needed

- Please fill out the Design Checklist or provide all pertinent information in written form, along with a floorplan sketch with overall dimensions including ceiling height, opening sizes and placement. Also include any additional information such as inspiration photos or photos of existing space if applicable.

- Note if openings are cased and thickness of trim.

- Include location of any specific cabinets (trash pullouts, rollouts.), appliances, and/or accessories.

Drawings provided are based on information solely provided by the contractor and or homeowner. All dimensions, cabinet sizes, accessories, trim, wall, window, and cabinet location are to be confirmed and verified by the contractor or homeowner. Any changes made after the receipt of these plans are the responsibility of the contractor or homeowner.

Designs and renderings will be as accurate as possible, but errors can occur or exact visuals are not available in the design program. Please review and confirm drawings carefully with actual field conditions, your client, and manufacturer specification guide. All drawings provided are an artistic representation of the general appearance of the design. They are not meant to be an exact rendition and may not exactly match the specification guide. Chosen fixtures and materials will be represented as closely as possible, but if textures and models are not available, generic items will be used.

All quotes provided are an estimate only. Final list pricing will be on the order confirmation from the cabinet manufacturer.

Origin Design Co. is not liable for any errors, miscalculations, or other issues arising from lack of review and verification by the contractor or homeowner. Any pricing shown on cabinet item lists will be list price and is an estimate only. Final list pricing will be on the order confirmation sent from the cabinet manufacturer. Origin Design Co. does not have any responsibility for ordering any cabinetry, changes to the cabinet confirmation received from the factory, or the installation thereof. Origin Design Co. may place cabinetry orders if Order Authorization form has been provided.

Rates:

$80.00 per hour billed at half hour increments, with a minimum of $160.

- Includes floor plan, elevations, perspectives, and color renderings

- Plans are sent via email.

- All changes/revisions must be submitted in writing via email. - Lead times for new designs is 7 business days and lead time for revisions is 4 business days unless otherwise communicated.

- Payments can be made either by check or credit card through ‘Square’. Checks payable to: Origin Design Co.
